I usually start with a thorough diagnostic assessment . I also like to find out what interests students have so that I can plan lessons around them individually as far as possible. If it were literacy I would usually do a Woodcock assessment. For other writing/ reading skills I would ask for an essay to look at to get me started on where "gaps" in understanding or skills lie.
